Unit summary

To summarise, this unit has evidenced that integrated care goes beyond the medicalisation of human care to include social care needs, in order to produce integrated care systems with their own identity, structures, processes and resources that connect, and care for, people in need of care, their families and carers (Leichsenring 2012). Most importantly, the patient or service user is viewed as being integral to this process.
