Permission and Contact Form 2026/27

To ensure that you can fully participate in your course and for safeguarding purposes, we need to receive a completed Permission & Contact Form for you for the 2026/27 academic year. Please fill in the correct form below as appropriate:

Learning Support Referral form – all learners

All students (including returners) must complete a Learning Support Referral form. This gives you the opportunity to express any areas where you feel you may need support. Whether you need support or not, completing the referral is essential to ensure no student is overlooked and disadvantaged.

DBS Application – Early Years Education & Care, Health & Social Care, Sport and Public Services students only.

If you are studying on an Early Years Education & Care and Health and Social Care course, you will need to complete a DBS Application once enrolled.

Public Services and Sports students may also need to complete the process depending on your chosen placement.

DBS Application Information

As part of your college course, you are required to complete a mandatory placement. This placement will be arranged in collaboration with you, your course tutor, an employer, and the placement team.

Before starting your placement, you must complete an Enhanced Disclosure and Barring Service (DBS) check within the first six weeks of your start date at college.

Support and Guidance

You will receive support from college staff throughout the DBS application process. For more details, please refer to the following SWAY containing important Enhanced DBS Information:

The Placement Team will manage your DBS application. They are responsible for:

  • Creating your DBS account
  • Monitoring your application progress
  • Answering any questions you may have

Application Process

University of Derby Buxton uses First Advantage Know Your People (KYP) Online Disclosures to process DBS applications.

Cost: £57.82 (non-refundable)

You will receive an email from First Advantage KYP Disclosures to your college email address. Please check your inbox and SPAM folder. Follow the link in the email to activate your account.

Steps to Complete Your DBS Application:

  1. Access your DBS account using the link in the email from KYP.
  2. Complete your online application and make the online payment.
  3. The Placement Team will contact you to arrange a face-to-face meeting to verify your ID documents.

After Submission:

  • Your DBS certificate will be posted to your home address.
  • It is your responsibility to:
    • Check the certificate for accuracy
    • Keep it safe
    • Present it to your placement provider before starting your placement

Lost certificates cannot be replaced – you will need to repeat the application and payment process if your certificate is lost.

Frequently Asked Questions

Invitations will be sent to the email address you provided during your application. Please check your inbox regularly, including your junk or spam folder, to ensure you do not miss any important updates.

If you are under 18, your parent or guardian will also receive the email, provided their email address was supplied during the application process.

You should receive your invitation approximately one week before the enrolment event begins.

Dependent on waiting times, the process will usually take around 1-2 hours to complete. To help reduce waiting times we have allocated time slots.

If you are under 18 you must bring a parent/ guardian with you in order to enrol. Parents and Guardians are also welcome to accompany students aged over 18 if they wish.

If you have concerns about your results, please visit us at any time on the enrolment days, to speak to one of our friendly Information, Advice and Guidance team.

Yes, if you have received an invitation, you will need to complete your enrolment again for this new academic year.
