Permission and Contact Form 2026/27

To ensure that you can fully participate in your course and for safeguarding purposes, we need to receive a completed Permission & Contact Form for you for the 2026/27 academic year.

Learning Support Referral form – all learners

All students (including returners) must complete a Learning Support Referral form. This gives you the opportunity to express any areas where you feel you may need support. Whether you need support or not, completing the referral is essential to ensure no student is overlooked and disadvantaged.

DBS Application – Early Years Education & Care and Health & Social Care students only

If you are studying on an Early Years & Care and Health and Social Care course, you will need to complete a DBS Application once enrolled.

How long will it take me to enrol?

Dependent on waiting times, the process will usually take around 1-2 hours to complete. To help reduce waiting times we have allocated time slots.

Can I bring my parent/guardian with me?

Yes, you are welcome to bring your parent/guardian along with you.

I’m not sure I have got the grades, what should I do?

If you have concerns about your results, please visit us at any time on the enrolment days, to speak to one of our friendly Information, Advice and Guidance team.

I can’t make any of the enrolment dates on the invite, what should I do?

Please email fecurricsupport@derby.ac.uk who will look at making alternative arrangements to sort your enrolment.

I enrolled last year, do I still need to enrol this year?

Yes, if you have received an invitation, you will need to come onsite to complete your enrolment again for this new academic year.
