Occupational health/NHS research passports
Pre entry Students
If you're about to become a student on one of the following professional programmes:
- Advanced Diploma in Nursing Studies
- BSc (Hons) Diagnostic Radiography
- BSc (Hons) Occupational Therapy
- MSc Occupational Therapy
- MA Art Therapy
- MA Dramatherapy
- Or a NHS Research student
You are required to complete a health screening programme in preparation for your clinical placements. This includes:
- Vaccination programme
- Health questionnaire
Vaccination programme - To prevent unnecessary vaccinations we need you to submit written evidence of any vaccinations and any blood tests you have already had for
- Hepatitis B
- MMR (Mumps, Measles and Rubella)
- Varicella (Chickenpox)
- BCG (TB vaccination)
This evidence should be obtained from your GP practice (ask the receptionist for a computer printout). If you have previously worked for the NHS ask the occupational health department for a copy of your immunisations and submit both prior to starting your programme.
The health questionnaire will be sent to you in an email (email address submitted on UCAS form) a month before you start. This is a confidential health questionnaire which is assessed by our Medical Officer. Any issues arising from this questionnaire will be discussed with you at your first screening appointment when you start your programme. A schedule of screening appointments will be arranged to enable you to have all the immunisations and tests you still need for your clinical placement.
