If you're unable to open the form you need you can download Adobe (Acrobat) Reader or simply contact any Student Support Centre and we'll send a copy out to you.

If you're a joint honours student and you want to make a change to your chosen pathway you'll need to make an appointment to see a joint honours academic counsellor.

External sponsorship
If your employer is paying your tuition fees you'll need to submit a sponsor letter.
This must:
- Be on company headed paper
- Be signed by your employer
- Detail the amount they wish to pay and for which academic year (one year or all years of your course)
- Contain your full name and student number.

Please note:
- Your sponsorship may not cover all of your tuition fees, so you may still need to make a payment to cover your personal contribution
- We can't accept sponsor letters from overseas organisations without prior approval from our Finance team.
University of Derby staff
If, as a member of staff, you're being financed by your department to undertake a programme with the University of Derby, you must submit a Staff Sponsorship Letter to cover the tuition fees incurred.
This template must:
- Be printed onto University of Derby headed paper
- Have all of the grey boxes accurately completed to fully detail the support granted for your programme of study
- Be signed by your Line Manager, Budget Manager and yourself.
Please note
- A new staff sponsorship letter is required for each academic year.
- Sponsorship by your department may not cover all of your tuition fees, so you may still need to make a payment to cover your personal contribution.
