Research associated with taught programmes must have ethical approval. Below are subject specific forms, if your subject is not listed then please complete the generic form and submit to your Independent Studies Supervisor or Co-ordinator.

Staff Ethical Approval Forms
Research activities undertaken by all staff must have ethical approval from their Subject Research Ethics Committee (SREC). Please complete the below form and return it to the Chair of your SREC either by email or post.
