Learning Beyond Registration

Learning Beyond Registration (LBR) is a vital part of developing competent, capable health service practitioners prepared to deliver a dynamic, flexible, quality, client-focused service. If you're employed by the NHS in the East Midlands then you may be eligible for fee-free access to our LBR modules and/or programmes. You'll need to supply evidence of approval from your line manager and Trust Education Lead when you apply.

About this module

As identified by the Department of Health document 'Supporting People with Long Term Conditions' (DOH 2005), there is a strong imperative for community based practitioners to reduce the number and frequency of hospital admissions in relation to acute exacerbations of chronic disease.

It is with this in mind that this module will provide the practitioner with the necessary skills and knowledge to empower patients in the management of their condition. It will enable the practitioner to engage with social, political and economic factors that challenge and support new ways of working.

Learning outcomes

On completion of the module you will be able to:

  1. Critically analyse the role of the practitioner in the management of long term conditions and the promotion of self care. 
  2. Demonstrate understanding of progression of the identified chronic condition by interpretation of clinical data, to support the treatment of disease processes. 
  3. Identify and work collaboratively with both multidisciplinary colleagues and the wider arena of community care.
