Waste Management Form

As a waste contractor to The University of Derby we would like you to provide details of the following:

  • Waste Management Licences
  • Environmental Permits
  • Exemptions
  • Carrier and/or Broker Licence
  • Hazardous Waste Premises Code


Please fill in the following form as comprehensively as possible.

The Environmental Manager, University staff and both internal and external auditors will have access to this information if deemed necessary.

The data will be retained for three years and reviewed on an annual basis

The data is being requested to ensure compliance with current waste legislation
